Medication Review
Medication review is a structured, critical examination of a patient’s medicines with the objective of reaching an agreement with the patient about treatment, optimizing the impact of medicines, and minimising the number of medication-related issues, and reducing waste. An appropriate health professional should carry out the medicine review. The review should consider:
The person and family/carers concerns, questions, or problems with the medicines
The person and family/carers views and understanding about their medicines
All prescribed, over-the-counter, and complementary medicines that the person is taking and what they are for
The safety of the medicines, how well they work for the person, how appropriate they are, and whether the medicine is in line with national guidance
If the person has any risk factors for developing adverse drug reactions
Any monitoring that is required
